Cleveland, Ohio clinic performs US’s first face transplant

Thursday, December 18, 2008

A team of eight transplant surgeons in Cleveland Clinic in Ohio, USA, led by reconstructive surgeon Dr. Maria Siemionow, age 58, have successfully performed the first almost total face transplant in the US, and the fourth globally, on a woman so horribly disfigured due to trauma, that cost her an eye. Two weeks ago Dr. Siemionow, in a 23-hour marathon surgery, replaced 80 percent of her face, by transplanting or grafting bone, nerve, blood vessels, muscles and skin harvested from a female donor’s cadaver.

The Clinic surgeons, in Wednesday’s news conference, described the details of the transplant but upon request, the team did not publish her name, age and cause of injury nor the donor’s identity. The patient’s family desired the reason for her transplant to remain confidential. The Los Angeles Times reported that the patient “had no upper jaw, nose, cheeks or lower eyelids and was unable to eat, talk, smile, smell or breathe on her own.” The clinic’s dermatology and plastic surgery chair, Francis Papay, described the nine hours phase of the procedure: “We transferred the skin, all the facial muscles in the upper face and mid-face, the upper lip, all of the nose, most of the sinuses around the nose, the upper jaw including the teeth, the facial nerve.” Thereafter, another team spent three hours sewing the woman’s blood vessels to that of the donor’s face to restore blood circulation, making the graft a success.

The New York Times reported that “three partial face transplants have been performed since 2005, two in France and one in China, all using facial tissue from a dead donor with permission from their families.” “Only the forehead, upper eyelids, lower lip, lower teeth and jaw are hers, the rest of her face comes from a cadaver; she could not eat on her own or breathe without a hole in her windpipe. About 77 square inches of tissue were transplanted from the donor,” it further described the details of the medical marvel. The patient, however, must take lifetime immunosuppressive drugs, also called antirejection drugs, which do not guarantee success. The transplant team said that in case of failure, it would replace the part with a skin graft taken from her own body.

Dr. Bohdan Pomahac, a Brigham and Women’s Hospital surgeon praised the recent medical development. “There are patients who can benefit tremendously from this. It’s great that it happened,” he said.

Leading bioethicist Arthur Caplan of the University of Pennsylvania withheld judgment on the Cleveland transplant amid grave concerns on the post-operation results. “The biggest ethical problem is dealing with failure — if your face rejects. It would be a living hell. If your face is falling off and you can’t eat and you can’t breathe and you’re suffering in a terrible manner that can’t be reversed, you need to put on the table assistance in dying. There are patients who can benefit tremendously from this. It’s great that it happened,” he said.

Dr Alex Clarke, of the Royal Free Hospital had praised the Clinic for its contribution to medicine. “It is a real step forward for people who have severe disfigurement and this operation has been done by a team who have really prepared and worked towards this for a number of years. These transplants have proven that the technical difficulties can be overcome and psychologically the patients are doing well. They have all have reacted positively and have begun to do things they were not able to before. All the things people thought were barriers to this kind of operations have been overcome,” she said.

The first partial face transplant surgery on a living human was performed on Isabelle Dinoire on November 27 2005, when she was 38, by Professor Bernard Devauchelle, assisted by Professor Jean-Michel Dubernard in Amiens, France. Her Labrador dog mauled her in May 2005. A triangle of face tissue including the nose and mouth was taken from a brain-dead female donor and grafted onto the patient. Scientists elsewhere have performed scalp and ear transplants. However, the claim is the first for a mouth and nose transplant. Experts say the mouth and nose are the most difficult parts of the face to transplant.

In 2004, the same Cleveland Clinic, became the first institution to approve this surgery and test it on cadavers. In October 2006, surgeon Peter Butler at London‘s Royal Free Hospital in the UK was given permission by the NHS ethics board to carry out a full face transplant. His team will select four adult patients (children cannot be selected due to concerns over consent), with operations being carried out at six month intervals. In March 2008, the treatment of 30-year-old neurofibromatosis victim Pascal Coler of France ended after having received what his doctors call the worlds first successful full face transplant.

Ethical concerns, psychological impact, problems relating to immunosuppression and consequences of technical failure have prevented teams from performing face transplant operations in the past, even though it has been technically possible to carry out such procedures for years.

Mr Iain Hutchison, of Barts and the London Hospital, warned of several problems with face transplants, such as blood vessels in the donated tissue clotting and immunosuppressants failing or increasing the patient’s risk of cancer. He also pointed out ethical issues with the fact that the procedure requires a “beating heart donor”. The transplant is carried out while the donor is brain dead, but still alive by use of a ventilator.

According to Stephen Wigmore, chair of British Transplantation Society’s ethics committee, it is unknown to what extent facial expressions will function in the long term. He said that it is not certain whether a patient could be left worse off in the case of a face transplant failing.

Mr Michael Earley, a member of the Royal College of Surgeon‘s facial transplantation working party, commented that if successful, the transplant would be “a major breakthrough in facial reconstruction” and “a major step forward for the facially disfigured.”

In Wednesday’s conference, Siemionow said “we know that there are so many patients there in their homes where they are hiding from society because they are afraid to walk to the grocery stores, they are afraid to go the the street.” “Our patient was called names and was humiliated. We very much hope that for this very special group of patients there is a hope that someday they will be able to go comfortably from their houses and enjoy the things we take for granted,” she added.

In response to the medical breakthrough, a British medical group led by Royal Free Hospital’s lead surgeon Dr Peter Butler, said they will finish the world’s first full face transplant within a year. “We hope to make an announcement about a full-face operation in the next 12 months. This latest operation shows how facial transplantation can help a particular group of the most severely facially injured people. These are people who would otherwise live a terrible twilight life, shut away from public gaze,” he said.

At least eight dead as building collapses during construction in China

Sunday, July 6, 2008

At least eight construction workers have died and thirteen more are injured after a building collapsed during construction in Qiuzhigou Village, a suburb of China’s Wuhan City. It is unclear how many more if any are trapped as the project’s coordinator fled and is being sought by police.

The collapse occurred at 5 p.m. local time yesterday and reduced the four-story structure to a five-metre pile of rubble. The building was a private residence and was illegal as authorities had not been informed of it. Its location in an area accessible only via narrow alleys is hampering search and rescue efforts as heavy equipment such as cranes is having difficulty reaching the scene.

Around 100 rescuers continue to pick through the rubble in the rain. An investigation has been launched.

Automobile manufacturer Toyota triples annual loss prediction

Automobile manufacturer Toyota triples annual loss prediction

Sunday, February 8, 2009

The Japanese car making company Toyota has announced that their predicted profit loss for 2008 has tripled from their previous estimate. The company reports the loss after demand for its vehicles dropped. In December 2008, Toyota estimated its full year operating loss to be 150 billion yen (US$1.65 billion). Now the company has tripled that number, forecasting a 450 billion yen (US$4.95 billion) loss. This would be the first yearly loss at Toyota in 70 years.

The firm also said that it predicts its global sales to fall by 17.87% to 7.32 million vehicles sold, compared to last year’s 8.91 million vehicles sold. Overall for 2008, Toyota’s car sales in the United States were down 15.4%, but that number was down from 2007 in which sales dropped 18%. For the month of January alone, Toyota’s sales fell 31.7% compared to the overall U.S. sales loss of 37.1%.

As a result of the loss, 17 of the company’s 75 production lines worldwide, will be reduced to only a single shift of workers. The company also announced a full closure of all their Japanese plants for a total of 14 days between January and March 2009.

Toyota’s boss Katsuaki Watanabe described the loss as happening only “once in a hundred years”.

In January, the Japanese Nikkei newspaper said that Toyota was thinking of firing 1,000 Northern American and British workers, all of whom hold full-time positions in the company. The paper quoted Toyota’s Executive Vice President Mitsuo Kinoshita as saying that “outside of Japan, we intend to make every possible effort to protect the jobs of our employees.”

California Supreme Court revives High School Exit Exam

California Supreme Court revives High School Exit Exam
August 5th, 2021 in Uncategorized | No Comments

Wednesday, May 24, 2006

The on-again, off-again requirement that California high school seniors must pass the High School Exit Exam is back on again, after the California State Supreme Court today granted the state’s request that the court stay a lower court’s decision to stop schools from requiring the test.

Lawyers for State Superintendent of Schools Jack O’Connell asked for the stay after Alameda County Superior Court Judge Robert Freedman ruled May 12 that the test requirement was unfair and unconstitutional, and forbade the state’s schools from using it as a graduation requirement. O’Connell’s lawyers also asked the Supreme Court to hear their argument that the Court should endorse the test and overturn Freedman’s ruling, but the Court sent the case down to the 1st District Court of Appeal for hearings.

“I am extremely pleased that the Supreme Court has reinstated the California High School Exit Exam as a condition for graduation,” O’Connell said after the decision was announced. “As a result, school districts can continue their graduation exercises as planned before this litigation began.”

Arturo Gonzalez, lead counsel for the students who challenged the test in court, told the James Logan Courier, “Our clients are disappointed in today’s 4-3 split decision of the California Supreme Court. However, this fight is not over. We intend to seek immediate relief in the court of appeals in San Francisco. We are hopeful that oral argument can be scheduled in time to obtain an order that would allow the Class of 2006 to graduate.”

O’Connell said he’s instructing school districts to withhold diplomas from those who haven’t passed the test. “I will communicate with districts today to ensure they are aware that the exit exam is a graduation requirement,” he said. ” Students who have worked hard to pass this exam will be given a diploma that signifies their mastery of essential skills in reading and math. I hope that this decision will give students in the class of 2006, their parents, and their schools certainty.

California Gov. Arnold Schwarzenegger praised the court’s decision. “”Today’s ruling is a victory for the children of California and for our future as a state,” said Schwarzenegger. “The exit exam ensures that our schools are living up to their responsibility by giving our students the skills and the knowledge they need to succeed in college and in the workplace. Postponement would have deprived us of the best tool we have to measure how well schools are doing their job.”

The ruling affects about 70 students at James Logan High School who haven’t passed either the math or language arts portions of the test, or both. As many as 47,000 are affected statewide.

“It’s crazy, I want to cry,” said Zuhal, a senior who hasn’t yet passed the test after she heard the news. Then she started crying.

Raymond, a senior who has had trouble with the math portion of the test, said he had made plans for graduating based on Freedman’s banning of the test. “I’m really disappointed,” he said. “I feel really bad. They should cancel it right now. Graduation day is too near.”

Both students took the test earlier this month, but won’t know if they passed until August 15, said Ray, too late to receive a diploma with their classmates.

“The Supreme Court did not say that Judge Freedman was wrong. Four justices simply questioned whether allowing our clients to graduate was the appropriate remedy,” said Gonzalez, “We intend to demonstrate that the relief was proper. If the constitutional rights of our children are violated, we cannot punish them further by depriving them of a diploma that they have rightfully earned by passing all required courses.”

O’Connell said students who haven’t passed the test would have more opportunities. “We will not give up on the students who are still struggling to pass,” O’Connell said, ” They will continue to be given every option to master those skills they will need in order to succeed beyond high school.”

O’Connell said he looked forward to arguing the case in the appellate court. “”In sending this case to the Court of Appeal, the Supreme Court has provided us an opportunity to argue the merits of this case,” he said.

O’Connell, who wrote the law when he was a state legislator in 1999, said the test is an important ingredient in California’s school reform efforts. “We know, and look forward to sharing with the court, that as a result of the exit exam, our students have studied harder and learned more than they would have otherwise,” he said. “Schools, administrators, school board members, parents, and even policy makers here in Sacramento paid more attention to the needs of students who were falling behind their peers.

“Today’s court action marks an important step for school accountability. I will continue to fight to make sure every student who graduates in California has the skills necessary to succeed,” he said.

Thai anti-government leaders escape capture

Thai anti-government leaders escape capture
August 4th, 2021 in Uncategorized | No Comments

Saturday, April 17, 2010

In a new setback to the Thai government’s efforts to ease mounting political tensions, protest leaders escaped from their hotel Friday after security forces arrived to arrest them.

One, Arisman Pongruangrong, climbed down three floors using a rope, and was rushed away by supporters thronging the building. The police say Arisman led recent rallies at the national parliament, the election commission, and satellite transmission bases.

Officials earlier Friday said the government is preparing to arrest people linked to violent clashes with security forces last Saturday.

Deputy Prime Minister Suthep Thaugsuban, Thai security chief, announced the raid on the hotel as it was unfolding, saying that a Special Forces unit had closed in around a downtown Bangkok hotel.

“As I am speaking, the government’s special team is surrounding the SC Park Hotel, where we have learned that there are terrorists and some of their leaders hiding,” Suthep said.

A spokesman for the governing Democrat Party, Baranuj Smuthararaks, says arrest warrants have been issued for those suspected of being involved in the violence. Some of the suspects have been identified from photos taken during the clashes.

“Right now the government’s focusing on issuing warrants for acts of terrorism by the people who fortunately have been captured in action [in photos] by both the local and international media,” he said.

According to the New York Times, as many as five protest leaders had been staying at the same hotel as Pongruangrong. Those leaders later appeared together at a protest rally in Bangkok.

“I would like to thank all of the people who saved me,” Arisman said. “You have saved democracy.”The government says armed men infiltrated protester ranks Saturday and fired on troops trying to disperse a rally. Five soldiers and 19 protesters died during the clashes.

Thailand is facing its most severe political crisis in almost 20 years. The anti-government movement, led by the United Democratic Front against Dictatorship or UDD, demands that the government step down and call fresh elections.

UDD supporters, known as red shirts, have held protests in Bangkok for more than a month.

The UDD largely supports former Prime Minister Thaksin Shinawatra, who was ousted in a coup in 2006 and remains overseas to avoid a jail sentence for corruption. Mr. Thaksin has strong support among the rural and urban poor, as well among some sections of the army and police.

Some parties in the governing coalition want to set a clear time frame for elections to ease tensions. But the government says it will only call elections once the political situation has cooled.

In early May, Wikinews extended an invitation to Vitaly Filipchenko, an independent candidate in the 2021 New York City mayoral election, set to take place November 2nd, alongside other candidates. Filipchenko answered some questions about his policies and campaign during a phone interview.

Filipchenko, registered on the New York City Campaign Finance Board as Vitaly A. Filipchenko, is the first Russian candidate for New York City mayor, being born in Tomsk, Siberia in 1973, according to news agency Sputnik. He has since naturalised as a United States citizen. According to the web site, Filipchenko has been educated in road construction and maintenance and owns a moving services company; he describes himself on his web site as a “small business owner”. On his web site’s platform page, he says that “[m]y English may not be perfect – but my platform is.”

Incumbent Democrat mayor Bill de Blasio, who won re-election in the 2017 New York City mayoral election by 66.5%, cannot run for a third term under term limits. As of April 28, 22 candidates are currently running, the majority of whom are also Democrats. Ahead of the June Democratic primary for New York City mayor, a poll conducted May 23 and 24 by WPIX and Emerson College of 12 Democratic candidates with a margin of error of 3.2 per cent has former commissioner for the New York City Department of Sanitation Kathryn Garcia and Borough President of Brooklyn Eric Adams leading with 21.1% and 20.1%, respectively.
