Step 5: The Rehab Process

This is where you get into the actual rehab itself. The key component is having been prepared for this stage of the process prior to actually starting any work, which is why the steps leading up to this are so important. Doing all your due diligence is key to a successful project. Before starting any type of work its time to pull permits and get inspections coordinated. You will want to have one last meeting with the general contractor (GC), tradesmen, carpenters, etc in order to go over the project and to make sure everything is in order. All the tradesman, carpenters, etc. need to know that the GC is in charge and that they need to refer to him for any construction questions. The GC answers to you. There are five major milestones for the permits and rehab process in general. The milestones are clean-out, frame-out, rough out, insulation and finishing. You begin by cleaning or gutting the property, as needed depending on your scope of work. Then you will proceed by framing out all the rooms and marking every aspect of the layout such as tubs, sinks, toilets, stove, phones, etc. so that the plumbers and electricians know exactly where to rough in. Next the electrician will come in and hardwire with new wire, cable smoke detectors, etc. The plumber will do the plumbing in kitchen and baths, the heating (HVAC) specialists will rough out the heating/cooling system duct work, baseboards, etc. When the rough inspection is complete then all the insulation is put in and then that is also inspected. Once the insulation inspection has passed you can now sheetrock, tape, sand and get ready for trimming out the property. Once trim is done its time to paint. Once the paint is done the electrician and plumbers come back to trim out or drop fixtures. The last thing to always do is flooring and carpet. Once thats all done its time to get your final Certificate of Occupancy (COs). Your last round of inspections to obtain your CO follows the same order of the rough inspections. First all of the subcontractors must get their final inspections okayed before the final building inspector comes to give a CO.

Step 6: Contract Closeout

Once the job is complete its time to do a walkthrough with a final punch list. There will be things that the contractor missed or need to be touched up or re-addressed. Make notes on the punch list of everything that needs to be finished up before they receive their final payment. Once theyve complete that final punch list and youve approved the final product they will sign the unconditional waiver of lien and they get their final payment. Getting the lien waiver signed, a copy of the final CO and completing the punch list is crucial. Make sure not to give the contractor their final payment until those items are taken care of.

Step7: Final Touches

Now its time to critique the house as if you were a potential buyer. The first thing to do is get the house cleaned. You need to make sure that the house is not dirty when buyers come to look at it. The second thing to do is stage the house. Since the house is vacant you want to stage it to visually optimize the space and highlight each room. Staging a house should not cost you more than $500. It will easily pay for itself by getting the investment back ten fold, so dont underestimate what staging can do. After the property is cleaned and staged go back with your camera and take after pictures. It will act as a documented copy for your files. A team of eight transplant surgeons in Cleveland Clinic in Ohio, USA, led by reconstructive surgeon Dr. Maria Siemionow, age 58, have successfully performed the first almost total face transplant in the US, and the fourth globally, on a woman so horribly disfigured due to trauma, that cost her an eye. Two weeks ago Dr. Siemionow, in a 23-hour marathon surgery, replaced 80 percent of her face, by transplanting or grafting bone, nerve, blood vessels, muscles and skin harvested from a female donor’s cadaver.

The Clinic surgeons, in Wednesday’s news conference, described the details of the transplant but upon request, the team did not publish her name, age and cause of injury nor the donor’s identity. The patient’s family desired the reason for her transplant to remain confidential. The Los Angeles Times reported that the patient “had no upper jaw, nose, cheeks or lower eyelids and was unable to eat, talk, smile, smell or breathe on her own.” The clinic’s dermatology and plastic surgery chair, Francis Papay, described the nine hours phase of the procedure: “We transferred the skin, all the facial muscles in the upper face and mid-face, the upper lip, all of the nose, most of the sinuses around the nose, the upper jaw including the teeth, the facial nerve.” Thereafter, another team spent three hours sewing the woman’s blood vessels to that of the donor’s face to restore blood circulation, making the graft a success.

The New York Times reported that “three partial face transplants have been performed since 2005, two in France and one in China, all using facial tissue from a dead donor with permission from their families.” “Only the forehead, upper eyelids, lower lip, lower teeth and jaw are hers, the rest of her face comes from a cadaver; she could not eat on her own or breathe without a hole in her windpipe. About 77 square inches of tissue were transplanted from the donor,” it further described the details of the medical marvel. The patient, however, must take lifetime immunosuppressive drugs, also called antirejection drugs, which do not guarantee success. The transplant team said that in case of failure, it would replace the part with a skin graft taken from her own body.

Dr. Bohdan Pomahac, a Brigham and Women’s Hospital surgeon praised the recent medical development. “There are patients who can benefit tremendously from this. It’s great that it happened,” he said.

Leading bioethicist Arthur Caplan of the University of Pennsylvania withheld judgment on the Cleveland transplant amid grave concerns on the post-operation results. “The biggest ethical problem is dealing with failure — if your face rejects. It would be a living hell. If your face is falling off and you can’t eat and you can’t breathe and you’re suffering in a terrible manner that can’t be reversed, you need to put on the table assistance in dying. There are patients who can benefit tremendously from this. It’s great that it happened,” he said.

Dr Alex Clarke, of the Royal Free Hospital had praised the Clinic for its contribution to medicine. “It is a real step forward for people who have severe disfigurement and this operation has been done by a team who have really prepared and worked towards this for a number of years. These transplants have proven that the technical difficulties can be overcome and psychologically the patients are doing well. They have all have reacted positively and have begun to do things they were not able to before. All the things people thought were barriers to this kind of operations have been overcome,” she said.

The first partial face transplant surgery on a living human was performed on Isabelle Dinoire on November 27 2005, when she was 38, by Professor Bernard Devauchelle, assisted by Professor Jean-Michel Dubernard in Amiens, France. Her Labrador dog mauled her in May 2005. A triangle of face tissue including the nose and mouth was taken from a brain-dead female donor and grafted onto the patient. Scientists elsewhere have performed scalp and ear transplants. However, the claim is the first for a mouth and nose transplant. Experts say the mouth and nose are the most difficult parts of the face to transplant.

In 2004, the same Cleveland Clinic, became the first institution to approve this surgery and test it on cadavers. In October 2006, surgeon Peter Butler at London‘s Royal Free Hospital in the UK was given permission by the NHS ethics board to carry out a full face transplant. His team will select four adult patients (children cannot be selected due to concerns over consent), with operations being carried out at six month intervals. In March 2008, the treatment of 30-year-old neurofibromatosis victim Pascal Coler of France ended after having received what his doctors call the worlds first successful full face transplant.

Ethical concerns, psychological impact, problems relating to immunosuppression and consequences of technical failure have prevented teams from performing face transplant operations in the past, even though it has been technically possible to carry out such procedures for years.

Mr Iain Hutchison, of Barts and the London Hospital, warned of several problems with face transplants, such as blood vessels in the donated tissue clotting and immunosuppressants failing or increasing the patient’s risk of cancer. He also pointed out ethical issues with the fact that the procedure requires a “beating heart donor”. The transplant is carried out while the donor is brain dead, but still alive by use of a ventilator.

According to Stephen Wigmore, chair of British Transplantation Society’s ethics committee, it is unknown to what extent facial expressions will function in the long term. He said that it is not certain whether a patient could be left worse off in the case of a face transplant failing.

Mr Michael Earley, a member of the Royal College of Surgeon‘s facial transplantation working party, commented that if successful, the transplant would be “a major breakthrough in facial reconstruction” and “a major step forward for the facially disfigured.”

In Wednesday’s conference, Siemionow said “we know that there are so many patients there in their homes where they are hiding from society because they are afraid to walk to the grocery stores, they are afraid to go the the street.” “Our patient was called names and was humiliated. We very much hope that for this very special group of patients there is a hope that someday they will be able to go comfortably from their houses and enjoy the things we take for granted,” she added.

In response to the medical breakthrough, a British medical group led by Royal Free Hospital’s lead surgeon Dr Peter Butler, said they will finish the world’s first full face transplant within a year. “We hope to make an announcement about a full-face operation in the next 12 months. This latest operation shows how facial transplantation can help a particular group of the most severely facially injured people. These are people who would otherwise live a terrible twilight life, shut away from public gaze,” he said.

The US Food and Drug Administration (FDA) has issued proposed calorie labeling rules requiring most retail food vendors to display the calorie counts in items on their menus and menu boards. The proposed rules, issued Friday and expected to be finalized in 2012, would apply to most restaurants, snack bars, vending machines, coffee shops, drive-through restaurants, and convenience and grocery stores.

The US Congress required the rules in the health-care reform law passed in 2010. The rules proposed by the FDA must undergo a public comment period before they are finalized and take effect, said Michael R. Taylor, Deputy Director for Foods at the FDA.

The proposed regulations pertain to businesses devoting more than 50 percent of their floor space to the sale of food or that consider themselves restaurants, specifically food-selling chains with at least 20 stores nationally. Included are candy stores, bakeries, and ice-cream parlors.

The FDA’s proposed guidelines specify that chains post the calorie counts of foods and drinks on menus and menu boards or next to the food item, such as at a salad bar. The menu is to prominently exhibit the calorie content of each item in a way customers can see easily, giving them the same information packaged foods prepared at home currently provide. The information must be displayed in “clear and conspicuous” print and colors.

Giving consumers clear nutritional information makes it easier for them to choose healthier options that can help fight obesity and make us all healthier.

Many cities and states have passed laws requiring calorie labeling on menus, beginning with New York City in 2008. California implemented a similar law in January, although many counties are waiting for the release of the federal guidelines before they begin enforcement. Some fast-food chains there, such as McDonald’s and Starbucks, are displaying calorie counts on menus in some of their stores.

The rules are intended to curb the national obesity epidemic since, according to FDA estimates, one third of the calories people consume yearly come from food eaten out. In a statement issued yesterday, Kathleen Sebelius, Secretary of Health and Human Services said, “Giving consumers clear nutritional information makes it easier for them to choose healthier options that can help fight obesity and make us all healthier.”

Excluded from the rules are businesses whose primary product is not food sales but that sell it, such as bowling alleys, airports and airplanes, amusement parks, hotels and movie theaters. Alcohol is also excluded.

Latest figures from the US government say that the country’s economy gained 290,000 jobs in April, the largest single-month gain since March 2006.

According to the Labor Department, excluding temporary workers for this year’s census, the number of non-farm jobs increased by a seasonally adjusted 224,000.

The unemployment rate, meanwhile, increased from 9.7% to 9.9%; the apparent contradiction is because 195,000 people who previously gave up searching for jobs started looking for work again, entering the labor force and augmenting the jobless rate.

The increases were larger than expected by economic analysts; according to a survey done by MarketWatch, economists believed the country would have added 185,000 jobs for last month.

President Obama commented on the numbers earlier today at the White House, saying: “This is the largest monthly increase in four years […] we’ve now seen job growth for four months in a row. These numbers are particularly heartening when you consider where we were a year ago, with an economy in freefall.”

San Jose, California —Del Borgsdorf, the city manager of San Jose, California announced his resignation on Tuesday, citing “personal reasons” and the “political environment” for his decision. While he denies it is was a factor in his decision, the resignation comes one day after Santa Clara County District Attorney George Kennedy announced his office will investigate the city’s contract with Norcal Waste Systems for possible criminal prosecution. Borgsdorf has been criticized by the city council for not informing the council of the $11 million rate hike promised to the company by mayor Ron Gonzales.

Borgsdorf’s resignation follows the resignation of other city officials; Stephen Haase, the city’s planning director, and John Bjurman, the city’s director of information technology, resigned earlier in the week. In addition, the city council formally censured mayor Gonzales last week, and asked for formal apologies from Borgsdorf and city attorney Richard Doyle over the Norcal contract. In addition to the waste removal scandal, the city government has been criticized for a no-bid contract with Cisco Systems to provide IT equipment in the new City Hall; the Cisco deal cost city taxpayers over $3 million in equipment costs and construction delays.

In the ruling, Taiwan’s Constitutional Court, which contains 14 judges, ruled in favor of extending marriage rights to same-sex couples. The Court also stated that, if the legislators miss the two-year deadline, same-sex couples will still be allowed to marry per ruling, which as things now are would make Taiwan the first Asian country to legalize same-sex marriage. One of the plaintiffs was Chi Chia-wei (???), a gay rights advocate since the 1980s who has campaigned for marriage rights since 2001.

The ruling received mixed reactions. Taiwan’s LGBT Family Rights Advocacy celebrated the ruling. However, groups like Coalition for the Happiness of Our Next Generation have opposed same-sex marriage and, as reported by Taiwan News, protested the ruling outside the Court, asked that the ruling be invalidated, and asked the country’s president Tsai Ing-wen to resign. Before becoming the nation’s first female president in 2016, Tsai campaigned for the legalization of same-sex marriage.

Lawmakers in Taiwan passed the first draft of a bill to legalize same-sex marriage in December 2016. The bill would replace “male and female parties” with “two parties” in Article 972 of the Civil Code concerning marriage. Anti-ruling protests have interfered with the passage of the bill, however.

Taiwan’s capital, Taipei, holds the largest gay pride marches in Asia, reports the Human Rights Campaign.

The People’s Republic of China — mainland China — continues to claim sovereignty over the self-governing Taiwan. In China, homosexuality was decriminalized in 1997 and delisted from the Chinese Psychiatric Association’s list of psychiatric disorders in 2001. However, of roughly 30,000 LGBT and intersex people, surveyed in China by Peking University, United Nations Development Programme, and the Beijing LGBT Center, most were found to be closeted, with only about 5 percent living openly.

Twenty-two countries around the world currently legally endorse same-sex marriage, including the United States and Great Britain.
